Health 360 Care Plan Creator

Upload the lab packet or paste the source results once. Health 360 drafts the provider lab interpretation and the patient Integrated Lab Results & Health Blueprint from your BHW templates, then leaves both open for clinical review.

Draft-first safety: Generation never publishes a plan. The provider reviews the source-grounded lab interpretation and every patient-facing action before approval. Only an approved, versioned Blueprint can be sent, and the patient must verify their identity before the portal displays it.
Care plans & Medicare AWV: Program actions, home monitoring, prevention-plan items, and the Medicare wellness visit type stay inside the same Health Blueprint. They are generated from BHW templates, reviewed by the clinician, and can be downloaded only after approval.
