TCM / Discharge Worklist
Upload the CRISP ENS Panel Details export and it surfaces the inpatient & observation discharges that are Transitional Care Management candidates (99495 / 99496) — with the 2-business-day call clock and the 7-day (99496) / 14-day (99495) face-to-face windows. ED visits and current admissions are listed separately for outreach.
Secure retained workflow. The CRISP file is parsed locally, then retained through the authenticated BHW
cloud service shared with CrewHQ. Desktop alerts contain counts only—not patient details. Decision support only:
TCM requires an eligible discharge to a community setting, an interactive contact within 2 business days, and
the face-to-face visit within the window. The 2-business-day clock ignores federal holidays—verify against the
calendar. Confirm eligibility before billing.
